HOW TO APPLY FOR AMP MEMBERSHIP

1. Obtain, fill up and sign the AMP membership application form;

2. Make sure you check the appropriate box indicating mode of payment;

3. Prepare 3 pcs. 1" x 1" colored ID pictures (white background);

4. Submit or mail the completed application form with pictures together with cheque payment to:

5. Upon submission of application form, pictures and cheque payment, the AMP membership ID will be processed and may be picked up from the office or mailed to your address.
