Dear Applicant,<\/p>\n

Congratulations and thank you for applying to be a member of AMP.\u00a0 You are two steps away in completing your\u00a0application process.\u00a0 The next step is to have you insured with our partner, PhilAm life Insurance by filling out the insurance form. Please \u00a0click on the link below, fill out the necessary information, print and sign.\u00a0 After which kindly take a photo or scan the signed form and email it to us at musiko@amp.org.ph<\/a><\/p>\n

Step 3 \u00a0Payment procedure<\/strong><\/p>\n

For new members, please pay the one time initial fee of Php 650.00.<\/strong>\u00a0 This fee shall cover your processing fee, ID fee and your insurance premium.\u00a0 Your welcome kit (Members ID, PhilAm life Insurance policy, sticker, bagtag) shall then be delivered to you either by messenger or via a courier service.<\/p>\n

For existing members, the annual fee is Php 500.00<\/strong>. \u00a0This fee automatically insures you for the year.
\nShould you wish to pay in advance, we offer discounted rates<\/em>.\u00a0 The rates are Php 950.00 for two (2) years and Php 1,350.00 for three (3) years. \u00a0Please make sure that your annual fee is updated to enjoy the benefits as a member of AMP and stay protected.<\/p>\n

You can deposit your payment at any BPI branch nationwide.\u00a0 Please deposit to:<\/p>\n
